must be accompanied by the Membership Application Information / Privacy Statement Form. Send both forms to the
EDAA with the correct payment.
If you wish to apply
for membership for part of the year you must contact the EDAA for a pro-rated fee application form.

Employers wishing to register as users of the EDAA Employment Registry services must complete and return the Employer
Statement of Understanding. Once on file the employer can access the services at any time by faxing in the Request for
